The funeral home’s role

NYC Health: 'When a funeral home is involved, the quickest way to get a death certificate is to order it from the funeral director who made the funeral arrangements. The funeral director can order death certificates for you up to one year after the death.'.

Mail applications must be signed in front of a notary. If the cause of death shows 'pending', the certificate can be exchanged within 90 days of issue without paying the $15 again. VitalChek is 'the only vendor authorized' by the NYC Health Department.

How many copies to order

Most banks, insurers, pension plans and government agencies each want their own certified copy. Count the accounts and policies before you order, because ordering more later means another application and fee. How many death certificates you need has a worksheet.
