The funeral home’s role
Utah allows families to care for their dead without a licensed funeral director (Utah Code 26B-8-120); families that do so must file the death certificate themselves, within five days of death and before final disposition.
If the applicant does not answer a request for more information within 90 days, the fee expires. Online orders go through the state's own site (secure.utah.gov/vitalrecords).
How many copies to order
Most banks, insurers, pension plans and government agencies each want their own certified copy. Count the accounts and policies before you order, because ordering more later means another application and fee. How many death certificates you need has a worksheet.
